Salary Slip Format
A monthly salary slip format with the standard earnings and deductions layout — PF, professional tax, TDS and net pay.
A salary slip is the employee's monthly proof of income and the employer's record of statutory deductions. Banks ask for it on every loan application, visa officers on most applications, and new employers at every switch — which is why even a two-person company should issue one each month.
The format below uses the standard two-column layout: earnings on the left, deductions on the right, netting to the amount actually paid. Keep components consistent from month to month, because basic salary drives PF and gratuity, and the HRA figure decides the employee's HRA exemption at tax time.
When to use this format
- Issuing monthly pay slips to employees along with salary credit
- Giving an employee income proof for a loan, visa, rental or job application
- Recording PF, ESI, professional tax and TDS deductions month by month
- Setting up payroll records for a new company's first hires
Key clauses and what they do
- Employee identification
- Name, code, designation, department, PAN and UAN — ties the slip to statutory records.
- Paid days & LOP
- Days paid and loss-of-pay days — the basis on which the month's salary is computed.
- Earnings column
- Basic, HRA and allowances — the gross before any deduction.
- Deductions column
- PF, professional tax, TDS and ESI — what the employer withholds and deposits.
- Net pay & words
- The amount actually credited, restated in words to prevent alteration.
- Payment details
- Mode and date of payment, with the authorised signature (or system-generated note).

General format for reference — get it professionally drafted for your specific case.
Which deductions apply (PF, ESI, professional tax) depends on your establishment's registrations, employee count and the state — professional tax slabs in particular are state-specific. Align the slip's components with your payroll registers and statutory filings.

Frequently asked questions
Is an employer legally required to issue salary slips?
Wage-slip requirements come from labour law applicable to the establishment — and practically, every bank, visa office and future employer will ask the employee for slips. Issuing one each month with the salary credit is the safe default for any employer.
Why does the basic salary component matter so much?
PF contributions, gratuity and leave encashment are computed on basic (plus DA where paid). Keeping basic artificially low reduces those liabilities but also the employee's retirement accruals — set it deliberately, not accidentally.
When is professional tax deducted?
Only in states that levy it, per that state's monthly slab. The employer deducts it from salary and deposits it under the establishment's professional-tax registration.
Does a salary slip need a signature?
A physical slip is signed by the authorised signatory. System-generated slips carry a note that no signature is required — either practice is accepted, as long as the slip ties out to the payroll register.
